Cave of Death is the fourth level of Hell's Fortress. As the name implies, most of the level takes place within caverns, featuring new textures and giving the level a unique style of its own. There are several larger chambers with structures within them, as well as several pools of water and lava.

Secrets

  • #1) In the large cave where you have to activate three Buttons to open the exit, there is a Shootable Button located on one of the two walls surrounding the floor Button, not far from the Nailgun. It moves a big rock at the end of the tunnel to the right of the exit gate, revealing a Ring of Shadows and Shells.
  • #2) In the circular room with wooden walkways, dive in the water and look behind the wall opposite of the exit to find a Red Armor, Multi Rockets and Lava Nails.
  • #3) After the ride on the moving cage, look under the bridge planks for a Shootable Button which open a wall nearby containing a 100 Health, Cells and Shells.
  • #4) In the last room of the level you have to make your way to the exit on a collapsing wooden walkway over lava. At the beginning of the walkway there is a small metal platform on the left that will lift you up to a previously inaccessible other walkway with a Red Armor and two 25 Health. The lift only became active when you are right below the exit tube, so you have to walk all the way up to the tube and then jump back down at the beginning.

Glitches

  • A Shambler entity, "t399", is spawned in all skill levels except Easy and Normal, where the corresponding teleport is not spawned.
  • On Easy difficulty, a final enemy do not spawn because of a missing trigger.
